You must complete this registration yourself. C.R.C.P. 207.14(A)(1)(a) or 227(A)(1)(a). As part of your completion of your registration statement, you are required to personally certify certain information. C.R.C.P. 207.14(A)(2)(a)(1-5) or 227(A)(2)(a)(1)-(5). You may not delegate the completion of your registration to another.

*NOTICE: To cover the cost of processing a credit or charge card transaction, and pursuant to section 5-2-212, Colorado Revised Statutes, a seller or lessor may impose a processing surcharge in an amount not to exceed 2% of the total payment made for goods or services purchased or leased by use of a credit or charge card. A seller or lessor shall not impose a processing surcharge on payments made by use of cash, a check, or a debit card or redemption of a gift card.

Pursuant to C.R.S. ยง 5-2-212 a small credit card processing fee of 2% will be applied to all credit card transactions. This fee is necessary to manage the rising costs associated with accepting credit card payments. To avoid this fee, consider using an alternative payment method such as mailing in a check or money order, Automatic Clearing House (ACH) e-check, or debit card which will not incur this surcharge. Note that payments sent via mail or express delivery that are due by a date-certain will be processed as the date received in our office, not the postmark or date mailed.
